Summit-Guidance has provided an editable form (.pdf format) that you may open and edit online. Some browsers support this right in the browser others will require you to first export it as a .pdf to you computer. If you save it and attampt to open it on your computer, you will need to have Adobe Acrobat Reader installed. You should be prompted to install it if you do not alrady have it in place. You may also find it: Get Adobe Acrobat Reader. Once you open it on your computer, you may fill out the form and save it with your specific information. Once this is done, you may either fax the printed form or attach it in an email to [email protected].
